A data subject request is a form designed to be implemented to your privacy policy page so that visitors could ask for data access, rectification, and erasure. The form collects requests from visitors and stores it in an admin panel called data subject requests.
For this form, you can set up the basic template as well as a success and failure note.

Data subject request form
To edit the basic data subject request form, follow these steps:
- Go to Menu > Consent Manager.
- From the website picker, choose the website you want to work with.
- Navigate to Consent templates.
- Scroll down until you see Data subject request section.
- Click Edit template of the form.
- Click an element of the form you want to change and edit it.
- When done, click Save.
- Click View code snippet.
- Click Copy to clipboard and paste on your privacy policy page.
Data subject request — success note
To edit a success note for a data subject request form, follow these steps:
- Go to Menu > Consent Manager.
- From the website picker, choose the website you want to work with.
- Navigate to Consent templates.
- Scroll down until you see Data subject request — success note section.
- Click Edit template.
- Click the form element you want to change and edit it.
- When done, click Save.
Data subject request — failure note
To edit a failure note for a data subject request form, follow these steps:
- Go to Menu > Consent Manager.
- From the website picker, choose the website you want to work with.
- Navigate to Consent templates.
- Scroll down until you see Data subject request — failure note section.
- Click Edit template.
- Click the form element you want to change and edit it.
- When you’re done, click Save.
